Job Description

DURATION: Minimum 6 weeks Maximum 8 weeks

WAGE: $15.50/hour (minimum 30 hours up to 37.5 hours/week)

This position is created specifically for applicants to the 2023 Canada Summer Jobs program, and all applicants must meet eligibility guidelines as specified by Service Canada in order to be considered for the above position.

GENERAL DESCRIPTION

Under the supervision of the Social Worker, the Social Worker Assistant is responsible for providing assistance to all areas within the department including, but not limited to, work in collaboration with the other multidisciplinary departments and actively participate as a member of the multidisciplinary team.

MAJOR RESPONSIBILITIES/DUTIES:

  • Assist in the process for admission, discharge or transfer of Residents in the Long-Term Care Homes while complying with the policies and procedures of the Central East LHIN and the Ministry of Health and Long-Term Care. Processes and maintains records of Residents of Shepherd Village and assists them with their specific needs as they relate to their accommodation.
  • Assist the coordination of the LTC Resident and Family orientation to Shepherd Village.
  • Advocate on behalf of Residents and their families, focusing on the social and emotional impact of physical and mental impairment, preservation and enhancement of physical and social functioning, and promotion of physical and mental health towards and optimal quality of life, while building upon existing strengths, enhancing adaptations, and modifying risks that interfere with optimal quality of life.
  • Assist in assessing Resident and Family dynamics and needs.
  • Provide emotional support to Residents and Family members.
  • Assist in rectifying issues that impact both the Residents and other departments (i.e. non- payment of LTC accommodation fee, completion of government documents, contacting PGT, capacity assessments etc.)
  • Ensure adherence to all applicable laws and regulations including the Resident's Bill of Rights, where applicable and Shepherd Village Mission Statement and Policies.
  • Communicates with the Care Team throughout the Admission Process.
  • Assist in maintaining an information package that meets the Ministry’s criteria and regulations and internal Policies and Procedures.
  • As a multidisciplinary team member, participates in ongoing care planning through involving the Resident and Family as much as possible.
  • Performs other related duties as required.


Program Eligibility

What youth participants are eligible?

To be eligible, youth must:

  • be between 15 and 30 years of age at the beginning of the employment period*;
  • be a Canadian citizen, permanent resident, or person to whom refugee protection has been conferred under the Immigration and Refugee Protection Act for the duration of the employment**; and,
  • have a valid Social Insurance Number at the start of employment and be legally entitled to work in Canada in accordance with relevant provincial or territorial legislation and regulations.
  • The youth must be 15 years of age at the beginning of the employment period. The youth may be more than 30 years of age at the end of the employment period as long as the youth was 30 at the beginning of the employment period.

**International students are not eligible participants. International students include anyone who is temporarily in Canada for studies and who is not a Canadian citizen, permanent resident, or person who has been granted refugee status in Canada. Youth awaiting a refugee status ruling, as well as those who hold a temporary visitor visa, youth visa or work visa are ineligible. As the objective of the CSJ program is to support youth entering the Canadian labour market, the temporary nature of an international student’s time in Canada does not allow for a long-term connection to the labour market.

Duration and hours of work

Jobs must be full-time (30 to a maximum of 40 hours per week). If a youth is provided less than 30 hours of work for any week, their wages may be deemed ineligible for reimbursement unless consent has been granted from Service Canada for exceptional circumstances (e.g. to accommodate persons with disabilities).
