The recuperative program supports individuals who are homeless who require to recuperate and successfully transition back into the community after discharge from hospital.

Please note: These positions will be located and will report to the Bridge Housing location in Niagara Falls

Shift Schedule: May involve shift rotation on days, afternoons, evenings, nights, weekends and holidays in a 24/7/365 environment. Including 12 hour shifts.

Job Description

JOB SUMMARY


Under the leadership of the Recuperative Care Supervisor, this position assists individuals who are homeless with the supports they require to recuperate and successfully transition back into the community after discharge from hospital. The Recuperative Care Support Worker will be providing personal supports, homemaking services and care coordination to individuals who are experiencing chronic homelessness and may be living with severe and chronic mental illnesses and/or addictions issues, in accordance with the philosophy, objectives, policies and procedures of Community Services. They will perform and collaborate as a member of the interdisciplinary team which includes the Recuperative Care Supervisor, Intensive Case Manager, Manager Homeless Operations, Bridge Housing Shelter Supervisor and Medical Director.


QUALIFICATIONS


EDUCATION

  • Grade 12 diploma or equivalent.
  • Graduate of an approved Community College - Personal Support Worker (PSW) Certificate Course or equivalent education as permitted by FLTCA, 2021
  • Training in Gentle Persuasive Approaches (GPA) an asset.


KNOWLEDGE/EXPERIENCE

  • Minimum of 3 months of current experience in a Supportive Housing/Assisted Living Program, or related experience in delivering personal support services to individuals.
  • Knowledge of resident/client safety issues and demonstrated capacity to apply resident/client safety practices.
  • Experience working with Homeless population and/or vulnerable population an asset.


SKILLS

  • Must possess excellent communication, analytical, problem solving, and time management skills.
  • Must possess ability to analyze and assess client’s needs.
  • Must have an excellent rapport with people and demonstrated working ability to understand the unique needs of the homeless population.
  • Demonstrated flexibility and resourcefulness in achieving client independence and quality of life outcomes.
  • Demonstrated teamwork, problem solving, and leadership skills sufficient to deal with crisis situations.
  • Ability to handle confidential information with diplomacy and discretion.
  • Ability to work under pressure and in crisis situations.
  • Skills in addictions, housing, community outreach, concurrent disorders, and general aspects of physical health are an asset.
  • Proficient in use of computer applications.


SPECIAL CONDITIONS

  • The Program operates 24 hours per day, 7 days per week, and 365 days per year.
  • Work is performed independently within established work procedures, departmental procedures, and safety procedures, as well as within regional policies and procedures.
  • In accordance with the Corporate Criminal Record Check Policy, the position requires the incumbent to undergo a Vulnerable Sector Records Check.
  • Ability to work flexible hours including days, evenings, nights, weekends, and holidays.
  • Demonstrated experience working with individuals with severe mental illness and capable of maintaining objectivity in order to perform consistently with the philosophy and objectives of the Program.
  • Physically and mentally capable of performing the duties of the job and have an excellent rapport with clients.
