Job Title: Cardiac Nurse Practitioner

Primary Location: Bracebridge and Huntsville

Reports to: Executive Director of Cottage Country Family Health Team and Director of Utilization & MAOHT Clinical Coordination at MAHC

Status: 12-month part time contract with possibility of extension Three days per week

Salary: $55.00 to $62.50 per hour, with vacation plus HOOPP

Hours of Work: Part Time Three days per week

The Muskoka and Area Ontario Health Team (MAOHT) is a collaboration of Muskoka and area health and social/community service partners and includes representation from 13 organizations, reflecting primary care, home care, hospitals, community agencies, mental health and addictions, the District Municipality of Muskoka and patient, family and caregiver partners.

POSITION DESCRIPTION

CCFHT is recruiting for a part time nurse practitioner on behalf of the Muskoka and Area Ontario Health Team. The position will work in collaboration with Muskoka Algonquin Health Care and other local and regional partners to facilitate the development and support a Heart Function Clinic using the CorHealth Spoke, Hub, Node Model.

Including but not limited to:

PATIENT CARE

• Works within the legislated scope of practice of the Nurse Practitioner, determines the need for, orders and interprets reports of appropriate laboratory and diagnostic imaging.
• Collaborates with hospital internists and local primary care providers to enable comprehensive care and referral for congestive heart failure patients within Muskoka and Area.
• Conducts patient assessments and develops care plans for all referred CHF patients triage as appropriate to other care settings.
• Provides direct comprehensive care to patients with specialized and complex needs.
• Ongoing management of HF patients (NYHA Class II and III)
• Consultation and referral as appropriate to the RVH consultant and HF Clinic (Node)
• Performs the responsibilities of the position consistent with the operational policies of the CCFHT.

CLINICAL LEADERSHIP

· Ongoing development and optimization of clinic practices, policies and care pathways

· Planning and coordinating care amongst multiple disciplines including nurses, community paramedics and support staff

· Representation of clinic in associated committees or partnerships

QUALIFICATIONS

• Current registration as a Registered Nurse in the Extended Class [RN(EC)] without limitations from the College of Nurses of Ontario.
• Current certification in Advanced Cardiac Life Support required
• Additional education and certification in area of specialty preferred
• Membership in the Registered Nurses Association of Ontario (RNAO) or Nurse Practitioner Association of Ontario (NPAO) organizations
• Up to five (5) years of relevant experience preferred
• Including 3 years’ experience working in an acute treatment centre
• Experience in working in an advanced clinical practice role required, preferably as a Nurse Practitioner or a Clinical Nurse Specialist
• Experience in nursing research methodology preferred
• Valid driver’s license
