Anishnawbe Health Toronto is a fully accredited community health center which offers access to health care practitioners from many disciplines including Traditional Healers, Elders and Medicine People. We are recruiting a 2 Registered Dietitians to join our multidisciplinary team which addresses the social determinants of health. We have broken ground for our new home of Anishnawbe Health Toronto, an Indigenous Hub in the West Don Lands. We expect to be completed in December 2023. Duties

• Perform nutritional instruction and coaching in outreach workshops for the AHT Diabetes Prevention Program
• Assess group and individual nutritional status by gaining an understanding of food habits and preferences (socio-economic, psychosocial and cultural background) and the clinical profile
• Develop and implement diabetes screening educational sessions
• Develop and implement meal planning for Individuals, groups and families
• Develop educational materials and curriculum
• Facilitate workshops and events to meet the needs of our population and reflect our core values
• Monitor and manage program and organizational data bases

Requirements

• Registration with the College of Dietitians of Ontario and Dietitians of Canada
• 2 years’ experience in a community and/or family health team setting is an asset
• Certified Diabetes Educator

Salary

69000.00+ depending on expereince
