The Rexdale Community Health Centre (the “Centre”) is a multi-service centre providing primary health care, settlement, community social support and community development activities for women, children, families, youth, adults and seniors of diverse cultures.
The Centre is currently recruiting a part-time Registered Dietitian to work with a multi-disciplinary diabetes education team aiming at the prevention of diabetes and education of clients with diabetes on management of the disease. The Registered Dietitian will also participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.
RESPONSIBILITIES:
· Provide one-to-one counselling to clients with diabetes to assess client nutritional status by gaining an understanding of food habits or preferences, socio-economic and cultural background, psychological factors and medical profile
· Develop, implement and evaluate individualized nutritional care plans for clients with diabetes, considering literacy, language, cultural and psycho-social factors
· Plan, implement and evaluate community nutrition activities for both individuals and groups
· Act as a nutrition resource to the Centre staff, professionals in training community agencies and groups
· Ensures continuity of care by maintaining up-to-date medical records, monitoring clients on a regular basis and case conferencing with other members of the health care team as required
· Advocate within Rexdale community on behalf of community residents and participate in the development of policies relevant to issues relating to diabetes and nutrition within the community
· Manage statistic record of diabetes programs and services of the Centre
· Provide in-service education to Centre staff and other community agencies requesting diabetes education
· Promote effective staff functioning and teamwork by participating in committees of the Centre as requested
· Contribute to educational student placement, volunteer and new staff orientation
QUALIFICATIONS:
· Certified Diabetes Educator (CDE) certification preferred
· Thorough knowledge of proficiency in diabetes care
· Bachelor of Science Degree in Food and Nutrition and completion of an accredited Dietetic Internship program (preferably with a community focus or equivalent approved experience)
· Registration with the College of Dietitian of Ontario
· At least two years’ work experience as a Registered Dietitian in an ethno-cultural diverse community based health service organization
· Demonstrated ability to work effectively in a multi-disciplinary team environment
· Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues in diet and nutrition
· Excellent interpersonal and communication skills (i.e. verbal and written)
· Demonstrated ability to facilitate groups
· Experience in planning and implementing health promotion programs and activities
· Familiarity with client centered and self-management approaches to chronic disease prevention and management would be an asset
· Demonstrated commitment to community-health care
· Ability to work independently as well as part of a team
· Willingness to travel between main office, Satellite office and Mobile Diabetes Clinics as needed
· Flexibility to work occasional evenings and weekends
· Knowledge of the Rexdale community
· Ability to speak a second language(i.e. Somali, Spanish, Urdu, Punjabi, Tamil) is an asset
Salary Range: $33.82 – $40.75 per hour (depending on experience)
Hours per week: 14 hours per week
Starting date: ASAP
