Skypointe 2 Complex Mental Health Rehabilitation Program is a new Addiction and Mental Health Continuing Care program located in the community of Skyview in Calgary providing 24/7 care to 32 individuals. Team members collaborate with clients, family, care providers, other programs and agencies to provide quality, client-driven, culturally appropriate and integrated services. Utilizing principles of Patient Family Centered Care, Recovery, Trauma Informed and Psychosocial models of care, the Social Worker will work collaboratively with the interdisciplinary team (Outreach Workers, Clinical Nurse Educator, Recreation Therapist, Occupational Therapist, Nursing staff (RN, RPN, and LPN’s), Clinical Behaviour Specialist, Mental Health Clinician, Administration, Peace Officers, and Physicians) as well as partner agency staff to provide care to clients both on site and in the community. The facility offers craft areas, library, group facilitation, and a fitness area to support the rehabilitation of the clients. Under the supervision of the Manager of Addiction and Mental Health Continuing Care, the Social Worker is responsible for intervention and counseling services that includes psychosocial assessment, crisis intervention, complex transitions planning, assisting with capacity assessment and OPT/OPG applications, financial assessments, education, treatment, advocacy, resource finding, consultation, liaising with community agencies/programs, individual/family supportive counselling and group therapy facilitation. Within the facility and in the community, the Social Worker will support clients to meet their individual recovery goals.
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 1 year experience within the last 3 years in Acute Care or Community Addiction and Mental Health programs. Authorized to perform Psychosocial Interventions with the ACSW. Working knowledge of the AB Mental Health Act and Community Treatment Order Legislation and other relevant legislation. Understanding of the DSM-5 with demonstrated knowledge of complex mental health diagnosis, symptomatology, verbal de-escalation, and treatment interventions. Experience in conducting psychiatric and psychosocial assessment, treatment planning, crisis intervention, and group facilitation. Experience in various risk assessment i.e. Suicide risk, Homicidal risk, violent risk.
Preferred Qualifications:Minimum 3 years of experience in an acute/community mental health setting. Familiarity of Harm Reduction Principles, Psychosocial Rehabilitation, Patient and Family Centered Care, and Recovery Oriented practice. Knowledge of the Calgary Zone mental health housing and community resources. Understanding of financial programs including AISH, CPP, Senior’s benefits, Income support and other social services. Able to complete capacity assessments.
